Your signed enrollment form, and paid Enrollment Fee, will be your CONFIRMATION OF ENROLLMENT. The total program cost is $1570. The $500 Enrollment Fee, includes, the non-refundable $500 registration fee, book and phlebotomy supplies,online study guide prep, and National Certification Exam-review.
This leaves a balance of $1070 ($870 for military). In the case of your withdrawal, or dismissal from the program, before the start of the third day of class, all monies received, minus the $500 Registration Fee and $100 for each day taught (not necessarily attended by you), will be refunded to the registered student, you, within 45 calendar days following students’ dismissal, or written withdrawal. Withdrawals must be written or emailed. Withdrawals will not be accepted by text message or verbal means. No refunds, of any kind, will be given for withdrawals, drop-outs, terminations, or no shows, after the start of the third day of the program. But we might, however, work with the student to place him or her in a future class at the “retake” rate, of $800, after student completes all financial obligations towards the class/program for which he/she is initially registered for. HMTC may credit the $500 registration fee, towards a withdrawn student’s enrollment, should student re-enroll within 90 days from confirmed start date. IMPORTANT: If you drop-out, are terminated (for any reason), or just decide not to attend class after day-three, you will still be held responsible for, and legally liable for, any unpaid balance you owe to the school. Any Student Account balances left unpaid by the end of the program, without other payment arrangements with the Training Center, will be taken to court to recover any unpaid balances. All discounts granted when the payment plan was established will be rendered null and void and become payable. Any fees for court action are also payable as well. This will affect your future employment in any field, as most employers do pre-employment background checks. PLEASE DO NOT LET THIS HAPPEN! Communicate with the Director, or lead instructor, as we are here to help you. H.M.T.C. reserves the right to postpone or cancel any program or class, at any time, especially prior to the start-date, due to lack of enrollment. We will reschedule any postponed or cancelled class and will work to accommodate the students at times and dates suitable for both parties. Any change in class schedule, does not affect the students’ enrollment status or responsibility toward their financial agreement with the Training Center. Note: Training & certification does not constitute a promise or guarantee of employment. By signing this form, you attest to understanding the terms of enrollment. You warrant/attest that the information you’ve provided below, although voluntary, is true & correct to the best of your knowledge and any falsified information may negatively affect your chances of enrollment at, or graduation from H.M.T.C., or when discovered, may be grounds for dismissal from the program, with no refund. You also give permission to H.M.T.C. to take photos and video of your participation in class and post on social media & website for advertising purposes.
